Develoopmental Milestones (sample)

Birth to 2 months
Fine Motor Ability: Holds hand in fist; draws arms and legs to body when crying
Gross Motor Ability: Comforts with touch.  Alerts to high-pitched voices.
Sensory Ability: Unrecalled
Vocalization: Cries to express displeasure such as wet diapers, makes comfort sound during feeding as claimed by mother
 Normal growth and development.
2-4 months
Fine Motor Ability: Readily brings objects from hand to mouth.
Gross Motor Ability: Can turn from side to back and then return.  When
Prone, holds head and supports weight on forearms
Sensory Ability: Turns head to look for voices and sounds. Mother also observed that during these months the patient tries to reach things that was shown to her
Vocalization: Patient makes cooing sound, started to smile and laughs such when the mother communicates her
Normal growth and development.
4-6 months
Fine Motor Ability: Mouths objects; Grasps with hold hands ( palmar grasp) such  holds bottle
Gross Motor Ability: Turns from abdomen to back by 4 months and then back to abdomen  by 5 months; Head held steady when sitting; Supports most of weight when held standing.
Sensory Ability: Responds readily to sounds.
Vocalization: Able to say Ma and Pa
Normal growth and development.
6-8 months
Fine Motor Ability: Transfers objects from one hand to the other and was trying to reach persistently for toys 
Gross Motor Ability: Sits alone steadily without support by 8 months; Likes to bounce on legs when held in standing position
Sensory Ability: Recognizes own name when being called and responds to pick-a-boo game.
Vocalization: Was able to say Mama and Papa and other two-syllable words
Normal growth and development.
8-10 months
Fine Motor Ability: Picks up small objects
Gross Motor Ability: Crawls or pulls body along floor by arms; creeps by using hands and knees to keep trunk off floor; Pulls self to standing and sitting by 10 months
Sensory Ability: Understands words such as “no”
Normal growth and development.
10-12 months
Fine Motor Ability: Can turn pages in books as mother recalled
Gross Motor Ability: Walks holding onto furniture; Sits alone
Sensory Ability: Unrecalled
Vocalization: Was able to wave hands and said bye-bye

1-2 years
Fine Motor Ability: Scribbles’ spontaneously,
Gross Motor Ability: Before 2 years old, patient walks with mother at back holding her both hands. Likes push and pull toys
Sensory Ability: Identifies basic parts of body such as eyes, nose, mouth, hands and ears.
Normal growth and development.
2-3 years

Fine Motor Ability: Starts using spoon when eating.
Gross Motor Ability: Walks up and down stairs; Runs; Jumps from stair or step with both feet.
Sensory Ability: Inserts square block to square shape.

Normal growth and development.
3-6 years
Fine Motor Ability: Uses plastic spoon; Uses tumbler.
Indentifies shape such as circle, square, rectangle and triangle at age of 3 years. At age of 3 years, learns to identify colors. At age of 5 years, learn to color the picture.  Stops thumb sucking at age of 5 years. Stops using drinking feeding bottle. At age of 5, learns to choose own food such what will be the viand.
Gross Motor Ability: Rides at motorcycle. Patient started to play jumping rope when she was 6. Able to follow rules set by the father at home.
Late development of exercising independence for her mother always attends to the patient’s needs.
6-9 years
Physical & Motor Ability:
At 6 years old, uses scissors. At age of 7, started to defecate at Comfort Room and brushes teeth independently.  At 8 years old, can dress and undress self. At 9 years old, independently wash anal area after defecation. Front teeth are lost around age 8 years. She wanted to play especially during recess.
Mental: She is interested in drawing and coloring pictures and reading story books especially “Cinderella”. Presently, she could count numbers 1- 1100. Can define the differences and similarities of objects. Can understand the common cause of her hospitalization as evidenced by her statement, “Tungud kay gipaak kog lamok”.
Adaptive:
 Still taking bath with the help of mother. She seldoms help in doing household chores.
Admitted that she is afraid to get grade below 80. 
Play: Engage in dramatic play such dressing up dolls and a girl to girl play.

Late development of exercising independence because she still relies on her mother when dressing up and undressing.
